Hours 8 AM to 5PM with occasional weekend and evening hours
Travel To parent homes, Unity Council program sites, and community agencies
Salary Range DOE

Benefits Medical, Dental, and Vacation on the first of the month following 90 days of employment. Vacation accrual at a rate of two weeks per year for the first two years.
Program The Unity Council is a non-profit community development corporation
Summary committed to enriching the quality of life of families primarily in the Fruitvale District of Oakland. Its primary focus has been to create a healthier and safer community for families and residents by implementing and managing integrated programs addressing the economic, social, and physical development.
Responsibilities The duties and responsibilities include but are not limited to the following: Work with the Social Service Team to recruit families, maintain full enrollment, track attendance, service delivery, and to provide transition services;
Assist families in completing program documents and updates, including but not limited to, emergency information and food program eligibility;
Provide services through a resource and referral model;
Create an Individualized Family Partnership Agreement with each family to build upon existing strengths to develop and achieve familial goals;
At a minimum, meet formally twice a year and check in monthly with each family, to ensure appropriate follow-up to goals and emergency needs;
Maintain a case load of approximately 51 families;
Work with classroom teachers, content area specialists, managers, and community partners to provide integrated comprehensive family services;
Encourage families to attend workshops and program events;
Provide written and oral translation as needed;
Conduct home visits, as needed;
Maintain documentation for each family contact through S.O.A.P. charting;
Attend staff meetings, relevant trainings, and community resource meetings;
Support children and families by participating in meetings and program events;
Perform additional duties as assigned by supervisor or management.
Qualifications Skills, Knowledge & Abilities
Bachelor's Degree related to Social Services;
Minimum of one year experience working with low-income children and/or families or providing resources and referrals;
Caring, compassionate approach to working with children and families;
Ability to work harmoniously in a multi-cultural team;
Familiarity with Alameda County resources;
Valid California Driver's license, good driving record and proof of automobile insurance, or the willingness to conduct home visits and accompany families to off site resources using public transportation;
Awareness of safe lifting techniques for lifting children, supplies and furnishings and commitment to using preventive health measures in the classroom to prevent the spread of disease.
Bilingual Spanish/English required.
